Request for Dates 9/1/2022 - 12/31/2022 due November 1, 2021.
Spring 2023 will open March 2022.

For Collaboratives with special circumstances, please fill out only one form using a semicolon between the parish names, dates, and times. Examples include separate ceremonies on the same day, or consecutive weekends. 

The comments field is also handy for further explanation of your Collaborative situation.

Fields with an asterisk (*) are required fields.


Please check here if there will not be any Confirmations in your parish between September 1, 2022 and December 31, 2022.
Please check here if you will be going to another parish for Confirmation.

Important Note

No Confirmation Ceremonies may be held on:

- 11/13/2022 - 11/18/2022

Please choose 3 tentative dates and times for each DAY of Confirmation.

For example, if you are celebrating 2 Confirmations on 2 different days, you must submit 6 tentative dates; use a semicolon between the days, dates, and times.


Confirmation Details

Please do not contact the Bishops/Episcopal Vicars directly, as this causes confusion in setting up the Confirmation schedule.

Please request your Regional Bishop/Episcopal Vicar to be the minister of Confirmation in the Comments section, if he has not celebrated the Sacrament of Confirmation in your parish within the previous two years.

If your parish has fifteen or fewer candidates for Confirmation, please arrange, if possible, a joint Confirmation with a neighboring parish. (However, every parish still needs to return this form and make note of this arrangement, or if no ceremony at all is being celebrated.)

If your parish usually celebrates Confirmation during Lent, please have one of the three dates you submit be during the Easter Season.

If your Bishop or Episcopal Vicar will be celebrating Mass, then please remember that there are days when ritual Masses cannot be celebrated, such as the Sundays of Lent or Advent. Please check the Liturgical Calendar to determine if any of the days you are choosing fall into this category. This only means that the Mass for Confirmation and the readings cannot be used. The Mass of the day must be used with possibly one reading of the day being changed.

In order to alleviate the large number of Confirmations scheduled for the Spring, it would be helpful if parish staffs give consideration to having Confirmation in the Fall.
