Catholic Hospitals of the Steward Health Care System
The six hospitals of the former Caritas Christi health care system (St. Elizabeth’s Medical Center, Carney Hospital, Norwood Hospital, Good Samaritan Medical Center, Holy Family Hospital, and Saint Anne’s Hospital) are now a part of Steward Health Care System LLC., of Boston. The Catholic identity of the hospitals has been preserved and they retain an important connection to the Archdiocese of Boston. Each institution is dedicated to providing health care that affirms Christ's healing ministry in accordance with Catholic teaching and to serving the physical and spiritual needs of patients with the highest quality care, compassion, and respect.

This form, which has been approved by the bishops of Massachusetts, is for the purpose of appointing a health care agent and alternate to make health care decisions for an individual according to Chapter 201D of the General Laws of Massachusetts.
